Our skilled nursing services include wound care, IV care, pain management, and infectious disease care. Several offer stroke rehabilitation, on-site dialysis, total parenteral nutrition (TPN) and other types of specialized care.
Many Mid-Atlantic Health Care centers are equipped with Step Up Units™ that serve patients with more complex medical needs. These units often permit earlier discharge from the hospital and minimize the need to transfer patients to the acute care hospital.
To further improve patient care and staff communication, Mid-Atlantic developed Real Time Medical Systems (RTMS), a platform that gives staff an intuitive, real-time dashboard that displays patient data. When a patient has additional needs, the system immediately informs staff of the need for a care intervention — enabling them to proactively monitor, identify and treat the patient at the facility, while preventing hospital re-admission. Since implementation of this platform, Mid-Atlantic has seen a 50% reduction in hospital readmissions.
In 2013, Mid-Atlantic became one of the first companies to go live with a CMS Bundled Payment for Care Improvement (BCPI) Model 3 program. This program seeks to provide more coordinated care to patients at a lower cost to Medicare by bundling payments to post-acute providers in order to align incentives.
