Problem Statement

Too many people who are referred to preventive screenings or support services are either not followed-up with by staff or are followed-up with, but are not accessing recommended services.

Aim / Overall Goal / Desired Result

40% of referrals by HEO staff will follow-through with the referral by 6/30/2023.

Change Idea

Improve preventive care by working closer with clients to ensure they have all the tools needed to follow-through with referrals to preventive screening and/or support services.

1. Promote new idea to staff.  Kendra   June 1, 2022.
2. Begin implementation July 1, 2022.   All Staff
3. Record referrals and follow-through on existing HEO database.  All staff
4. Monitor progress monthly. Kendra  beginning Aug. 1, 2022
