A community collaborative developed from the Child & Adolescent Wellness Workgroup, to specifically focus on reducing childhood obesity by helping children develop healthy eating habits, be physically active, reduce screen time, and get adequate sleep. Medical providers include members from Wellspring Family Medicine, Mountain Laurel Medical Center, and Lovelight Pediatrics.

    This is a brief outline of our goals, strategies and the screening plan for our group. It is based on the strategies that Shelley uploaded. Please review those as well.     RFP that the Population Health, Innovation & Informatics Unit was awarded in December of 2020. We were able to innovate and create a well-being program, found at gogarrettcounty.com for the county instead of redesigning or adding structure to our Health Planning Council. An emphasis on reducing chronic disease, specifically diabetes is a major focus of the program.
