Mountain Laurel Medical Center would choose our DSME (Diabetes Self Management Education) program as a tracking measure. We already track patient successes and report them yearly to the AADE. Our clinical goal for this year is that all patients who complete the 10 class DSME program will lower their fasting blood glucose level 5%.

Shout out to Mitzi and Victoria from the Diabetes program at GRMC who let us know that they are actively holding classes (evidence- based lifestyle change programs) various target populations and what they need most are physician referrals!
