Child and Adolescent Wellness Workgroup 6 years, 5 months agoThis is the suggested parent permission form for the middle school youth. In the form, a side-by-side comparison of the parent and adolescent versions of the survey are mapped out. If we use something like this, the parents will be completely informed of the minor differences between the two versions. Please give feedback.

After reviewing these versions of the surveys (parent and adolescent), I see something we can improve upon.
The final question asks for choices from a list of problem-oriented reasons to speak to a health care provider.
I suggest we add some to this list that are wellness-oriented. For example:
