The Local Care Team is an inter-agency council designed to provide support to families. The collaborative group unites representatives of several child-serving agencies to serve families of children struggling with emotional, educational, mental and/or legal issues through inter-agency discussions and problem solving.

Local Care Team (LCT) Performance Measures Defined: Number of unique referrals received by the LCT during the reporting period- Referrals counted in this measure are youth-specific, not family-specific. Youth who were reported in previous reporting periods may be counted again if the LCT received a new referral. Not every referral will result in a youth review. Total number of LCT reviews completed during the reporting period- Reviews may be conducted in-person or virtually. They are attended by the mandated LCT members, youth's attorney where applicable, youth's parent and/or legal guardian, and the youth when appropriate. A “review” is one function of the LCT and there may be more than one review scheduled for each LCT meeting. Likewise, a referral may result in multiple reviews during the reporting period. A meeting or part of a meeting where the LCT discusses systemic gaps and needs not specific to a particular youth or family is not a review. Discussions involving siblings / other family members who have not yet been referred to the LCT are not counted. Number of inbound LCT cross-training and other activities completed- This measure pertains to activity including training and other presentations completed for the benefit of the LCT. (e.g. ‘Lunch and learns’ or presentations from community based organizations received by the LCT.) Number of LCT meetings- Number of meetings scheduled by the LCT Coordinator (in-person or virtual) to conduct reviews and/or administrative matters. Number of unique referrals that resulted in one or more reviews by the LCT during the reporting period- This number can only be equal to, or less than, the number reported in "Number of unique referrals received by the LCT during the reporting period" Number of outbound LCT outreach activities completed- This is a measure of activities that originate from the LCT coordinator or an LCT member acting on behalf of the LCT. This is LCT “marketing” to individuals and/or groups. A training is defined as an event (such as, but not limited to a workshop, meeting, outreach, etc.) in which the LCT coordinator/member is the presenter/speaker and provides information to others (e.g. agencies, caseworkers, etc., not the LCT) on LCT related-issues, LCT business practices, etc. Percent of mandated LCT representatives that attend at least 75% of the meetings- Numerator: Number of mandated LCT representatives that attend at least 75% of LCT meetings. Denominator: Number of mandated LCT representatives. This measure pertains to the individual attendance of each mandated member. Calculate the total number of meetings and the total number of those meetings attended by each mandated member. For example, if there were 10 meetings in the half-year period, and the mandated member, Sue, attended 5 of those meetings, her attendance is 50% with N=5/10. This measure is NOT a calculation of the group’s attendance of a meeting. Percent of all LCT reviews where the youth's parents (or legal guardians) attended Numerator: Number of all LCT reviews (new, follow-up and annual reviews) where the youth’s parents (or legal guardians) attended. Denominator: Total number of reviews completed during the reporting period. Calculate the total number of reviews and the total number of those reviews for which a youth’s parent/legal guardian attended/participated. For example, if there were 100 total reviews in the half-year period, and a parent/legal guardian attended/participated in 60 of those reviews, report 60% with N=60/100. Parent attendance can be in person or via simultaneous electronic means, such as telephone or video conferencing. _________________________________ Local Care Team performance measure data are collected semi-annually and annually. Performance measures utilize the Results-Based Accountability method to track and improve community health performance. Performance measures take into account how much we are doing, how well we are doing it and whether anyone is better off in relation to the Garrett County Local Care Team and its efforts. In FY 2024 (from July 1, 2023-June 30, 2024), the Garrett County Local Care Team (LCT) received 26 total new LCT youth referrals, and reviewed a total of 30 youth cases, including review cases. Both case reviews and new referrals increased in the second half of the FY, with 17 of the 26 new referrals being received between January 2024-June 2024 and 22 of 30 youth review cases being held in this same period. Attendance for mandated members at mid-year averaged at 72.9% and increased to 74.6% at FY end. At least one parent or legal guardian attended 100% of LCT youth review meetings in FY24. The LCT received 3 training sessions for FY24 and attended 6 Community Outreach Events.
