Local Care Team
Public Group active %sThe Local Care Team is an inter-agency council designed to provide support to families. The collaborative group unites representatives of several child-serving agencies to serve families of children struggling with emotional, educational, mental and/or legal issues through inter-agency discussions and problem solving.
Goal:
The Garrett County Local Care Team (LCT) ensures that children/youth and their families with intensive needs receive necessary support and resources to live a successful life.Strategy Description:
The Garrett County Local Care Team helps children and families to identify their individual needs and coordinate assistance through referrals to local agencies. The LCT initiates inter-agency discussions and problem solving for individual family needs as well as systemic needs. The LCT refers children and families to Care Management entities when appropriate as well as local and community resources. The LCT identifies and shares resource development needs with the Care Management entity, Local Care Teams and local Core Service Agencies/Behavioral Health Authorities concerning how to best help the family. The LCT can discuss a request for voluntary placement for a child with a diagnosed developmental disability or mental illness.Level of Change:

Data Narrative
Local Care Team performance measure data are collected semi-annually and annually. Performance measures utilize the Results-Based Accountability method to track and improve community health performance. Performance measures take into account how much we are doing, how well we are doing it and is anyone better off in relation to the Local Care Team. For the first half of FY22, three (3) new cases were referred to the Local Care Team with four (4) cases reviewed by mid-year. Attendance for mandated members at mid year was 77.8% with 100% of parents attending the scheduled meetings. One LCT training was held virtually for the Early Childhood Advisory Council with positive feedback and requests for promotional materials. For the first half of FY21, three new cases were referred to the Local Care Team with 3 cases reviewed at mid-year. The Local Care Team had 100% parental attendance for the meetings with 78% of mandated representatives present at each meeting. Two Local Care Team training sessions were held by mid-year FY21 where information regarding the Local Care Team and its purpose were shared along with promotional materials. These pieces of training were held for the Mental Health Education Task Force Committee and the Mental Health Advisory Committee of Garrett County. For the second half of FY21, four new cases were referred to the Local Care Team with 5 cases reviewed by the end of June. The Local Care Team had 100% parental attendance for meetings with 77.8% mandated representative attendance for the year. A total of 9 Local Care Team meetings were held in the second half of FY21 with 16 total meetings for the year. Two youth were referred to the Local Care Team requesting residential placement. These youth were alternatively served within the community due to placement requirements not being met by the client.
