Number of Referrals Received: measures the total count of new patient referrals sent to the home health agency within a calendar month, regardless of whether the patients are ultimately admitted.
Number of Monthly Admissions: measures the total number of patients formally admitted to the home health agency during a month
Number of Monthly Discharges to Community: measures the total number of patients who are discharged from the home health agency during the month having met their care goals and remaining safely in the community
Number of Monthly Recertifications: measures the total number of patients who are recertified for continued home health services and being deemed eligible for ongoing skilled care.
Number of Hospital Readmissions: measures the number of patients who are admitted to a hospital after starting home health services.
Number of Hospice Transfers: measures the total number of patients who transition from home health services to hospice care during a given month.
