Evaluation of 2017 CDSM workshops in Garret County in comparison to State of Maryland. Demographics of program utilization, both quantitative and qualitative information. Of special note: Garrett County is the only County that enjoyed 100% completion rate for the full six-week program in all workshops.
These Stanford University Patient Education Center's Chronic Disease Self Management Programs (evidence-based) are partially funded by the Administration on Community Living with technical assistance provided by the National Council on Aging. Fidelity to the community based program is monitored and insured.
