2017 Garrett County Chronic Disease Self Management Programs evaluation (323.5 KB)
Evaluation of 2017 CDSM workshops in Garret County in comparison to State of Maryland. Demographics of program utilization, both quantitative and qualitative information. Of special note: Garrett County is the only County that enjoyed 100% completion rate for the full six-week program in all workshops.
These Stanford University Patient Education Center's Chronic Disease Self Management Programs (evidence-based) are partially funded by the Administration on Community Living with technical assistance provided by the National Council on Aging. Fidelity to the community based program is monitored and insured. -
