Local Care Team referrals may be completed by families (self referrals) or by agencies on behalf of a family. Families should be made aware of any referrals as a Local Management Board staff member will be contacting the identified parent/guardian upon receiving a new Local Care Team Referral. The Local Care Team referral can be filled out as a google form linked here:
