The Garrett County Health Department's Home Health intake team will determine eligibility criteria is met prior to admission:

1. Home Health Intake Form to be updated as needed
   1. 48 hour admission window
   2. H&P reviewed
   3. F2F has the appropriate verbiage, signature, date
2. RN to review chart for appropriate orders
