The mission of our Hospice is to provide physical and emotional support to the terminally ill and their families in a home-like atmosphere where they can get relief from pain, improve their quality of life, and die with dignity.

Hospice patients never receive a bill for services rendered, so Hospice relies largely on donations from members of the community to ensure that our patient supply need are met.

With a new location recently opened in McHenry, Hospice now has 2 locations to allow us to better serve the community.

Data Narrative

Measure 6- Number of Veterans Served: Our social worker will work with veterans and their families to see if they may qualify for extra benefits, such as paid caregiving, through the VA benefit program and will help with the application process. Measure 7- Number of Patient Rollovers to Next Month: Number of patients that will continue into the next month on Hospice services. Measure 8- Number of Live Discharges: Describes the number of live patients discharged from Hospice services. This is usually due to an improvement of their illness. Measure 9- Other Discharges: Describes the number of patients that have passed away while on Hospice services Measure 10- Revocations: Describes the number of patients who have resigned themselves from Hospice services, often to re-seek aggressive treatment for their illness
