Goal:
increase the rate of patients over the age of 50 years old receiving the shingles vaccination, by providing vaccine education and eliminating the excessive cost factor by providing the patient with a Med Bank referral.
Strategy Description:
One on one education during Home Health visits about the benefits of the vaccine. Advise them to talk to their doctor. leave them with an informational brochure and also giving the a referral coupon for the Health Department's Med Bank where the vaccine is provided at no charge to the patient.
