Measure 1: Number of unduplicated guests who register through 360AccessHub.com/HelpA note about the trend decreasing; we want to shorten the line and get people connected to the resources that will help them reduce their need for food and nutrition security services. Measure 2: Number of registered guests who are served through the Population Health, Innovation, and Informatics Unit (PHII) within the Garrett County Health Department Measure 3: Number of registered guests who are served through the staff of Community Action, Inc. Measure 4: Members of Maryland Physicians’ Care (MPC) who are selected by MPC and are then served through 360 Access Hub Measure 5: Number of guests connected to other community resources by (PHII), including but not limited to well-being programs to reduce/manage chronic disease: Community Action services, Diabetes Education through GRMC, and MLMC, Diet Consults,  Dove Center, Dept. of Social Services, GOGarrettCounty.com, GoGarrettCountyPlus.com, Judy Center, LiveHealthyGC.com, Maryland Wellness, and other comunity supports Measure 6: Number of guests connected to other community resources by Community Action, Inc. staff including but not limited to well-being programs to reduce/manage chronic disease: Community Action services, Diabetes Education through GRMC, and MLMC, Diet Consults,  Dove Center, Dept. of Social Services, GOGarrettCounty.com, GoGarrettCountyPlus.com, Judy Center, LiveHealthyGC.com, Maryland Wellness, and other comunity supports Measure 7: Number of volunteers trained via the volunteer portal on 360AccessHub.com Measure 8: Number of hours donated by 360 Access Hub-trained volunteers Measure 9: Amount of cash donations received by Community Action for 360 Access Hub operations Measure 10: Number of special community events held at 360 Access Hub
