The Allegany County and Garrett County Hub Pilot has been invited to apply for Track 1 Funding  of the Consortium on Coordinated Community Supports in response to the Request for Applications (RFA) released by the Maryland Community Health Resources Commission (CHRC). The RFA and supporting documents can be found here: https://health.maryland.gov/mchrc/Pages/notices.aspx

As the Hub will be applying to the CHRC to become a full Community Support Partnership, service providers will be included within the application that the Hub submits. As outlined in the RFA, interested service providers should contact the Hub for more information. The contact for Allegany County and Garrett County is Fred Polce, [email protected].

The Hub would like to hold two Coordinated Community Supports Informational Meetings in order to review the requirements of interested service providers and answer questions from interested providers. Questions may be submitted ahead of the meeting(s) to [email protected]. 

The meetings will be held on Thursday, January 2, 2025 at 11am and Monday, January 6, 2025 at 2pm. The meetings will be held virtually via Google Meet. Meeting links are included below for your convenience. The Maryland CHRC will also be holding a second Frequently Asked Questions call for all potential applicants on Wednesday, January 15 at 10 am. More information can be found in the RFA. 

To Indicate Interest in the Community Supports Partnership: 

The Hub is requesting information from service providers interested in being included in the Allegany County and Garrett County Community Supports Partnership response no later than Friday, January 17th at 4pm.  

Please utilize the attached “Questions for Interested Providers” document for your response. Responses should be sent to Fred Polce at [email protected]. 

Also attached is a list of services currently available and identified needs within Allegany County Public Schools (ACPS) and Garrett County Public Schools (GCPS). Currently available services and needs have been identified through work completed by ACPS, GCPS, and the Hub. 

The Hub will consult the Scoring Rubric, found on page 34 of the RFA to score interested service provider proposals to the Hub. Note that the Local Education Agencies (LEAs) may also rank the interested service providers by priority and will be directly involved in the service providers selection process.
